Prior to Surgery Policy

If you have had any prior surgery within the last 6-12 months for the body part about which you are consulting, you will need to furnish records of that surgery. Please complete and sign the Medical Records Release from Other Facilities form on our Forms page and send it to your physician.

Once we have received and reviewed the records, we will contact you to set up an appointment with your physician.
