Office Appointments
We try to schedule your appointment at a time convenient for you. Please tell the receptionist why you are coming in, so we can schedule the right amount of time for your visit. Note that your initital visit may take up to one hour, to allow time for a complete history and physical.
When you phone to make your appointment, please be prepared to provide your address, phone number, social security number, insurance information, and complete medication list.
If you need to cancel your appointment, please let us know at least 24 hours in advance. Failure to cancel at least 24 hours in advance will result in a missed appointment fee of $75 (click here for our No-Show Policy).

Preparing for Your Appointment
- If you are a new patient, please fill out and bring with you the completed registration forms. Please click here for our Forms Policy.
- If you choose not to download the forms, please arrive at least 15 minutes before your scheduled appointment time, so you can fill out the necessary forms.
- Bring your insurance card and driver’s license or other form of ID.
- If you have had previous treatment for your current injury, bring copies of medical records, X-rays and any additional diagnostic results, if available. To obtain your records, you may print out the Medical Records Release form from our Forms page, fill it in and mail it to your physician. Please note you may have to pick up any actual diagnostic films (X-ray, MRI, CT, Bone Scan, etc.) and bring them with you.
PLEASE NOTE: In order for us to provide the best quality care, your initial appointment may be rescheduled if you are not able to provide medical records, X-rays and diagnostic results pertaining to this injury in advance.
